Appointments

• Service Location: The scheduling form will request a physical address for where services will be rendered. Please complete this form with accurate information that allows our team to identity the correct location. If a change is needed please contact us to avoid a re-scheduling or cancellation fee as described in this document.
• Deposit Policy: We request a full payment of services for mobile massages or event online appointment which will be applied to services rendered. Please note that in the event that we are not able to make contact for a scheduled appointment or if the appointment is rescheduled within short notice as described in the re-scheduling policy the payment amount will be forfeited and cannot be returned unless Pura Vida Therapy is contacted and mutually agrees to extenuating circumstances.
• Re-scheduling Policy: If the appointment time that was previously booked is no longer convenient please re-schedule your appointment from the email confirmation or by contacting us 12 hours before the start time of your appointment. If this proper notice is not received the payment amount will be charged as a re-scheduling fee.
• Event re-scheduling policy: Special events such as the San Francisco Bachata Congress or Denver Salsa Bachata Congress will also require at least 2 hour notification for a cancellation or refund to be made available. Due to the heavy volume of appointments during these events late cancellations or no-shows after this time do not qualify for a refund. Please use the information in our Contact Us page to reach us in the event of a need to cancel your appointment.
• Cancellation Policy: If an appointment is not previously needed please cancel your appointment from the email confirmation link or by contacting us 12 hours before the time of the appointment. If this proper notice is not received the full charge of your appointment will be charged as a cancellation fee.
• Minors: All children under the age of 18 are classified as minors and must have a parent and or legal guardian signature in their intake form. Children under the age of 12 must have a parent present during the scheduled session.
• Length of Session: We appreciate the opportunity to start and end services in the agreed upon time. Requests to extend sessions will be handled on a case by case basis and may incur additional costs which can be agreed upon while services are being provided.

Payments

• Full payments are due at the time of service being completed considering deposit amount. Payments can be made in the puravidatherapy.com site or in cash.
• Price Changes: Pricing is subject to change at Pura Vida’s sole discretion without prior written or verbal communication.
• Damages and Lost Items: Pura Vida is not responsible for damages due to factors outside of our control while in-home services are being rendered.

Privacy Policy

• Collected data: To provide our customers with a great experience our website uses features that collect information automatically like “cookies” that enable user login features or manually completed forms like “intake forms” which provide our staff with preferences for the services being rendered. If these cookies are rejected we ask that you do not use our website features and instead contact us for scheduling needs as the features may be limited in their functionality.
